- Right to information regarding the diagnosis and treatment
- Right to informed participation in decision involving his/her care
- Right to refuse treatment
- Right to consult with other specialist(s) at his/her own expense
- Right to explanation of the need for transfer elsewhere for continuity of care
- Right to be informed of hospital rules & regulations
- Right to suggest or express your grievances
- Right to explanation for all items in the bill
- Right to access to your clinical records as per hospital policy
RESPONSIBILITIES
- Providing accurate and complete information about your medical complaints, past illnesses, hospitalizations, medications, pain, and other matters relating to their health.
- Following the treatment plan recommended by those responsible for your care
- To accept the consequences in-case of refusal of treatment from your care given
- Paying your bills promptly
- Being considerate of the rights of other patients and hospital Staff
- To take care of your personal belongings
- Notifying a member of the health care team if you do not understand information about your care and treatment
- Reporting changes in your conditions or symptoms,including pain to a member of the health care team
- Keeping your scheduled appointments or to canel them in advance if at all possible
- Following the rules and regulations of the Hospital
